How To Apply For WIC

If you are interested in applying for WIC, contact your local health department.
Translation services are available in all languages.

At the time of your appointment you will need to bring in the information listed in the 4-Steps to WIC.

Step 1: Who Comes To The Apointment

Please bring the family members applying for WIC Services to the appointment. (It may be you, your child and/or your infant.)

Step 2: Proof Of Identity

You must bring at least (1) identification for yourself and (1) identification for your infant or child who is applying for WIC. Some of the proofs that WIC will accept are as follows:

For women applying and parents or caretakers:

Driver's License

Recent Paycheck / stub with name

Current  bill with your name on it

Current Car Registrations

Current Medicaid Card

Valid ID card with photo

Current student ID card with photo

Current Insurance card/policy

Military Identification U.S.

Current Medical Record

Current SC WIC ID card

Valid U.S. Passport

Marriage license

Valid Social Security Card

Current Voter Registration Card

Current Work ID with photo

 

For your infant and/or child:

Birth Certificate

Hospital Birth Card

Foster Child placement letter/notice

Valid Social Security Card

Current Medicaid Card

Client's school ID or record

Immunization record

NOTE: CITIZENSHIP AND IMMIGRATION STATUS ARE NOT REQUIRED FOR THE WIC PROGRAM.

Step 3: Proof Of Residency

You must bring in (1) proof of where you live. The document must show your address. (No P.O. Box numbers.)

Some of the proofs that WIC will accept are as follows:

South Carolina Driver's License

Recent Bill

Current Bank statement

Client's school record

Current Mortgage or Rental Agreement

Current Car Registration

Benefactor Letter (from a reliable third party such as a social worker, landlord, etc.)

WIC Appointment Letter (with street address)

Current Medicaid Card

Unemployment letter/notice or Worker's Compensation letter

Food Stamp Notice of Eligibility (An EBT card is NOT acceptable proof)

Recent Leave and Earnings statement (LES) for military personnel

Most recent Pay Stub

Foster child placement letter/foster parent award letter with amount received for foster care

Signed statement from your employer indicating gross earnings for a specific time period (weekly, bi-weekly, or monthly)

Court order or divorce decree showing alimony and/or child support payment

Regular cash contributions: signed/dated letter from the person (not living in the household) who regularly contributes income to the household

Self-employment: recent 1040 ES quarterly tax form, or company account books (records), or 1040 form for the past year

W-2 forms or income tax return for the most recent calendar year

Step 4: Income Documentation

You must bring in documentation of all income for everyone living in your household. The document(s) must be current (within the last 60 days).

Some of the proofs that WIC will accept are as follows:

Current Medicaid Card

Unemployment letter/notice or Worker's Compensation letter

Food Stamp Notice of Eligibility

 

Recent Leave and Earnings

 statement (LES) for military personnel

Most recent Pay Stub

Foster child placement letter/foster parent award letter with amount received for foster care
